We met Pamela in Season Four's opener "Lazarus Rising" when Bobby takes Dean and Sam to visit her.  She is a very old friend of Bobby's and conducts a séance to see who or what pulled Dean out of hell.  To do so she must touch the handprint that Castiel left on Dean's left shoulder.  In doing so she see Castiel's real face and has her eyes burned out of their sockets from the sight.
